Sportswomen of Colorado, Inc. will pay tribute to Colorado's outstanding female athletes and to persons who have contributed to girls and women's athletics at its 36th Annual Awards Banquet on March 7th, 2010. Honors will be awarded for performance in 2009. A media panel will select award winners from nominations submitted on the enclosed form and postmarked or faxed no later than December 21, 2009.

1. Fully complete and submit the online form below or click on the link below to download and print the form. If you choose to download print the form, when completing typing is preferred.

2. Within the form list the nominee's achievements in the given year specified.
3.If you have downloaded the form, mail to the address below. Also, you are encouraged to mail a maximum of two (2) letters of endorsement; these are limited to one page each and are optional. If you completed the online form and wish to mail the two letters of endorsement, please reference the nominee's name in a short cover letter. Please, mail to:

4. All nominations become the sole property of Sportswomen of Colorado, Inc.
5. Select the proper category for your nominee. A given performance must merit an award; therefore it is possible some categories will not be filled. Athletic awards will be based on performance during 2009, so a summary of 2009 achievements must be included.
a. Most awards will be given for outstanding achievement in specific sports, such as, but not limited to basketball, cross country running, cycling, golf, gymnastics, horsemanship, skiing, soccer, softball, swimming, tennis, track and field, and volleyball. High school, collegiate, amateur, professional, recreational and masters athletes are eligible.
b. A Coach of the Year and Pioneer Honoree also will be named, the latter for exceptional pioneering contributions to athletics, recreation or sports in general over a period of time.
c. At the Selection Panel's discretion, additional "Special Category" awards may be presented for activities in 2009 and/or over a period of time. Previous headings were Most Courageous, Comeback, Leadership, and Contribution to Sports, Recreation, Special Olympics and Most Inspirational.
